GEtting Your 1098-T

The contact information you had on file while enrolled will be used to mail IRS Form 1098-T (Tuition Statement), if one is applicable.

Note: 1098-T’s are prepared and mailed by a contractor.

If you need to update your contact information with Creative Hair School of Cosmetology, or you wish to see if your 1098-T was returned to the school, please email:

NEW!: You may also submit a Change of Information request online at https://www.creativehairschool.com/change-of-information.

Creative Hair School of Cosmetology does not email 1098-T’s. Duplicate 1098-T’s must be picked up in person or mailed.

GEtting Licensed

Step 1: Apply for State License

Visit the Michigan Department of Licensing and Regulatory Affairs (LARA) and go to APPLY FOR OR RENEW A LICENSE. Choose OCCUPATIONAL PROFESSIONALS. Choose COSMETOLOGY and then click on REGISTER YOUR ACCOUNT NOW. Complete the Login Information in order to create your account. After 3-5 business days, you will be able to register with PSI. The registration fee is $63.

Step 2: Complete the Examination Registration form

The Examination Registration form can be found at the end of the PSI Bulletin. A portion of the form must be brought in to be completed by the school!

Step 3: Register with PSI Exams

PSI Exams has updated their website!

To create an account with PSI Exams, go to https://test-takers.psiexams.com/micos/auth/register. Go to General Information for more information on how to book a test or request accommodations.

To schedule the test, go to PSI Exams and click on I’m a Test Taker. Go to the Find a PSI test search box and filter by industry (select “Barber and Cosmetology” in the dropdown menu) and state (select “Michigan” in the dropdown menu). Scroll down to the list of options below the search box and click on the correct test for your field of study. (For example, “MI Cosmetology” or “MI Manicurist.” Instructors program students should go to “MI Cosmetology and Related Occupations Instructor.”) Click on Sign in/Register.

After logging into your account:

• Schedule your first test attempt using the BOOK tab.

• Schedule any additional or Retake test attempts using the MANAGE tab.

The exam fee is $167 for both parts. The instructors test is $93.

Getting Work

While Creative Hair School of Cosmetology cannot guarantee employment upon graduation, we are committed to assisting every graduate seeking employment, both upon graduation and throughout their entire career. We are willing to help graduates with professionalism, resume development, interview preparation, and job-seeking skills so they are well equipped for the working world. We also work with various salon owners. Inquiries may be made in the school's administrative office.
